Understanding fibromyalgia and persistent pain

In persistent pain conditions the pain system itself has become more sensitive. The nervous system is doing its job of protecting you, just turned up too high and left on for too long.

That means the amount of pain you feel is real and it doesn't reflect damage in the tissue. It's a genuine, physical change in how signals are processed, and it's why the usual advice about resting and waiting doesn't help much.

What does help is bringing activity back at a level your system can accept, then increasing it slowly enough that it doesn't set off the alarm. That's a different skill from ordinary rehab and it needs a different pace.

What we know about the causes

  • Fibromyalgia is a recognised condition diagnosed by a doctor, usually after other causes have been ruled out.
  • It often follows a period of illness, injury or significant stress, though it can begin without an obvious trigger.
  • Poor sleep is both a symptom and something that makes everything else louder.
  • Long periods of reduced activity lower your tolerance further, which makes the next attempt harder.
  • Boom and bust patterns, where a good day gets overdone and costs the next three.

Managing the flares

  • Work to a level you can repeat on a bad day, and build from there.
  • Protect your sleep routine, because it has a bigger effect on symptoms than almost anything else.
  • Spread activity across the week instead of doing it all on the good days.
  • Keep some movement going through a flare, even if it's much less than usual.
  • Track what you did rather than how you felt, because feelings vary and data doesn't.
