Understanding arthritis and joint pain

Arthritis describes changes to a joint over time. In osteoarthritis, the most common form, the cartilage and surfaces of the joint change and the joint becomes more sensitive to load.

What surprises most people is how weakly those changes relate to pain. Scans of people with no symptoms whatsoever show the same findings that get blamed when someone is struggling. Pain is influenced by strength around the joint, how much you're doing, sleep, stress and a good deal more.

The joint doesn't wear out from use, which is the other thing worth knowing. Joints are nourished by movement. It's the doing less that tends to make things worse.

The different types of arthritis

  • Osteoarthritis. The most common form. Gradual changes to cartilage and joint surfaces, usually in knees, hips and hands.
  • Inflammatory arthritis such as rheumatoid arthritis, which behaves differently and needs medical management alongside physiotherapy.
  • Degenerative changes in the spine, which are extremely common with age and often symptom free.
  • Previous joint injury, particularly ligament or meniscal injuries, which can bring changes on earlier.
  • Long periods of doing very little, which reduces the muscle support a joint depends on.

Fewer flare ups

  • Keep moving on the stiff days. Joints do better with regular gentle load than with rest.
  • Build strength around the affected joints. Muscle is what takes pressure off them.
  • Pace your activity across the week instead of doing everything on a Saturday.
  • Stay on top of sleep and general health, because both affect how much pain you feel.
  • Keep some form of resistance training going as you get older. It protects far more than joints.
