Understanding back pain

Back pain is extremely common and it's rarely as sinister as it feels. In most cases there's no serious damage. Scans of people with no pain at all routinely show the same findings that get blamed when somebody is hurting.

What's usually happening is a mix of things. Tissue that has become sensitive, muscles that have stopped sharing the load evenly, and a nervous system that has got protective about certain movements. Add a stressful few months or some broken sleep and the whole thing gets louder.

Your back is strong. The problem is normally that it's been asked to do a lot without much support around it, or asked to do very little for a long time and then suddenly a lot.

What causes back pain?

  • Long stretches of sitting followed by a sudden burst of activity, like a weekend of gardening or a house move.
  • A lifting or twisting moment that started it off, even a fairly ordinary one.
  • General deconditioning. Backs cope far better when the hips, glutes and trunk are doing their share.
  • Poor sleep and a stressful stretch at work. Both turn the volume up on pain.
  • An earlier episode that settled without anything actually changing afterwards.

Stopping back pain coming back

  • Keep moving on the bad days. Gentle movement settles a back faster than the sofa does.
  • Build strength through the hips, glutes and trunk so your back isn't doing everything on its own.
  • Change position regularly if you sit for work. The best posture is the next one.
  • Ramp up gradually after a quiet spell rather than going straight into a full weekend of it.
  • Protect your sleep. It has more effect on pain than most people expect.
